Kokoro Connect DVD/BD
Because I can't find any BD releases for KokoroCo, I decided to compare the broadcast to the DVD release instead. I just love this show.
DVD includes eps01-02 + NCOP/ED + PVs.Sources:

>>> They made some changes on the text in the blackboard. I copied rori's translations for those changed text.
Also, the OP song used in the DVD/BD are different from the broadcast.
>>> Give it back already.
>>> They added a text on this one. The character that I can read is 明日= tomorrow.
>>> I'm sorry I was born.
>>> I love you, Yui. I love you.
>>> Sorry. I can't go out with you. Seriously.
>>> They made the rainbow more translucent, which, for me, looks better.
>>> I'm not sure if there were actually changes here, because the difference in colors might be a result of encoding.Part A
>>> They lengthened the scene in the DVD, and totally cut off that frame showing their bags.
>>> For some reason, they removed the keyboard in Inaban's laptop.
>>> Zoomed out.Part B
>>> There were a lot of redrawn scenes in the second half of the show. They were mostly fixes on how the characters were drawn. I didn't take screenshots of every redrawn parts because it will take me ages to put them up and there's really no need to, is there?
>>> They inserted a white scene to mark the start of Part B.
>>> There were two short scenes were they showed Inaban blushing, and both were changed. She looks less of a tomato, so I think it's a nice fix.PreviewEp02
>>> Again, fixes on the characters' faces/expressions. Also, the theme song animations are the same for both versions.
Unlike in Tasogare where the fixes in the first volume of the BD were also done in the broadcast, the OP/ED in the broadcast of Kokoro Connect didn't change. Well, I haven't checked beyond eps01-02 so I can't say for sure yet. Too lazy to check.
HorribleSubs rip Crunchyroll's streams, while rori used Tokyo MX's broadcast.
For the OP song, they originally used "Paradigm" performed by eufonius. They changed it to "Kimochi Signal" sung by Horishita Sayuri. If you're interested as to the reason, check this MAL discussion.
includes eps 03-05 and a drama CD.Ep03
>>> It's the same as with the previous episodes. There were a lot of fixes concerning the expressions of the characters, mostly facial. No background change noted. These applies to eps 04 and 05 too. I did not take screenshots of all the changes since it's too tedious to link each of them.
>>> I like how this has been redone. It just shows how much they paid attention to details as they prepare the show for the BD release. Notice the changing in the lighting? But did you also notice how they fixed the ear too?Ep04
>>> Don't mind her mouth.
>>> They were a bit inconsistent in how they drew Inaban's hair in the TV. Refer to this second ss: http://i.imgur.com/N1Sam.jpg
>>> Change in perspective / distance.
>>> Inaban is just so cute. Too bad I find the voice acting of her CV to be too exaggerated.Ep05
>>> Slight fix on Yui's hair and face shape.
>>> They forgot to animate Iori's hair as it flows with the wind.More notes
>>> For the BD, I re-encoded the raws from ANK-Raws.
>>> They removed the prologue scene in ep04. It's the scene in the infirmary. Then they used the prologue scene in ep05 as the epilogue in ep04.
>>> Since there's no more prologue, they went straight to the OP as with ep04.
>>> In the broadcast version, there's no ED in ep05. Instead, they inserted the song, along with the credits, as the show ends. In the BD, they have the usual ED, so no need to insert the song and credits.
>>> Lastly, you know how they show the title logo to mark Part A and B? In the broadcast, they kept it on as Part B starts, but in the BD, they removed it. If I'm not mistaken, this applies from ep02 to 05, and it might be the same for the rest of the episodes.
